Thoracic epidural analgesia provide effective pain control and more rapid patients rehabilitation after kidney and upper third ureter surgery
The effectiveness of postoperative analgesia in 60 patients after kidney and upper third ureter surgery were assessed. Thoracic epidural analgesia (TEA) by the local anesthetics provide more effective pain control then systemically administered opioids, that confirmed by the reduction of area under a curve of visual analogue scale on 70,5% at rest and 60,8% at movement (p<0,001). TEA during operation and in the postoperative period allows to achieve significant opioid-sparing effect and reduce total dose of fentanyl on 81,5% and promedol on 61,1% (p<0,001). Application of TEA lead to reduce the incidence of postoperative nausea from 50% to 20% (p<0,05), time of mechanical ventilation in postoperative period from 43,8±2,8 to 22,0±1,8 min. (p<0,01) and terms of hospitalization patients after surgery from 13,8±1,1 to 10,1±0,6 days (p<0,05).
